My first Helen's suicide mission.

EDIT: Lol. To clarify, this is not actually possible. What happened was....I don't know actually. A glitch obviously. My squadies simply ceased to be visible in the cutscene, they actually were present. Helen made it through with no fatalities. ;)

Sutori-Artifex is right just the do loyalty missions and upgrades, use common sense in assigning your squad members.
